Definition and Classification

Heart failure (HF) is a clinical syndrome characterized by symptoms and signs caused by a structural or functional cardiac abnormality, resulting in reduced cardiac output or elevated intracardiac pressures.

Stages (ACC/AHA)

Stage A

At risk, no structural disease, no symptoms. DM, HTN, obesity, family Hx.

Stage B

Structural/functional disease, no current or prior symptoms.

Stage C

Structural disease with current or prior HF symptoms.

Stage D

Advanced HF; refractory symptoms despite GDMT. Consider advanced therapies.

Clinical Presentation

Left-sided HF

  • Dyspnea on exertion (earliest)
  • Orthopnea, paroxysmal nocturnal dyspnea
  • Pulmonary crackles (rales)
  • S3 gallop (volume overload)
  • Fatigue, reduced exercise tolerance

Right-sided HF

  • Peripheral pitting edema
  • Jugular venous distension (JVD)
  • Hepatomegaly, ascites
  • Weight gain from fluid retention
  • Often secondary to left HF
